It’s time to give them a break.
Thanks to Spruce, we can stop asking them and start asking our Healthy Life Team (HLT) without even making a phone call. Through this easy-to-use app, we can get our questions answered, quickly make appointments and access referrals to in-network providers.
To get started, text the Healthy Life Center Scheduling Line at (877) 365-0051. You’ll receive a text back with a Mohawk-specific link to Spruce. Set up a new account with your personal information, and you’re ready to start text messaging our HLT through the app. Click here to see a step-by-step video showing how to download the app and get started.

Healthy Life Center Appointments:
“I’d like to make an appointment at the Dalton Green Street HLC the week of July 1 for my annual wellness exam.”
“When’s the next available appointment at the Dalton Green Street HLC. I’ve got a sore throat.”
Healthy Life Navigator Appointments:
“Hi! I’d like to talk to someone about stress.”
“I’d like to schedule some time to talk to a healthy life navigator.”
Ask medical questions:
“Hi. I think my ankle may be sprained. It’s swollen and bruised. Take a look at this photo, and let me know if I should come in for an exam.”
“Is muscle soreness normal after a shot?”
Find an in-network provider:
“Hi. I need to see a cardiologist. Can you recommend an in-network provider close to Muskogee, OK?”
