All Invited to Complete Voluntary Forms

In order to help Mohawk Industries comply with federal law, the Company is inviting all its current employees to voluntarily complete two self-identification forms. These forms will be mailed to all employees’ homes, and the Company is also making the forms available in this myMohawk article.

Mohawk is a contractor to the federal government, which means the Company sells products used in federal office buildings, military bases and other properties. Federal laws require all federal contractors, including Mohawk, to invite existing employees to self-identify whether they currently have or previously had a disability. Copies of the form which Mohawk is required to use is available by clicking HERE. You are not required to disclose anything about any disability. You have the option of not answering the form.

You may also disclose whether you are a veteran. A copy of the form for disclosing veteran status is available by clicking HERE. You have the option of not answering the form.

If you choose to answer either or both of the forms, please return the completed form to your Human Resources Department.

The forms are voluntary and confidential.

Completion of the invitation form is strictly voluntary. Any information you provide will be kept confidential and used only for purposes required by laws that apply to Mohawk because of the Company’s status as a federal contractor. Any information you choose to provide will not be made part of your personnel file and will not be used against you by Mohawk for employment purposes. Similarly, if you choose not to provide any information, your decision not to participate will not be used against you.
